CV For a Job in Audio Environment...

I'm preparing to write a new curriculum that i'm going to send to the local radio/tv station (national) to try and get a job there. I've been working for the same company for the last 15 years doing something completely different but have alsso been involved in many recording projects (recording & producing local bands demos) over the last 10 years or so in my spare time as well as playing numerous concerts with my bands over during the last 20 years. I also attended the SAE school for a year or so part-time.
I feel like i need a change and if i don't start now i never will (40 yrs old...mid life crisis...ha, ha).
I need some advice on how to get this new cv together...what information to include, what not to include...what you studio pros look for when reading cv's that are sent to you, etc...
I would be grateful for any info...
