Single-channel headphone amp HELP!

Hey all,

Here's the situation. I'm setting up a complicated(ish) wired/wireless IEM rig for my band and need some suggestions. In order to make it work for the guys on wired IEMs, I need some sort of headphone amp between the console output and them.

Long story short, I need three channels of headphone amp that is switchable to mono, has separate L/R inputs (preferably XLR), and isn't outrageously expensive. Pretty much EXACTLY like this:

Except not $289/ch.
