About to release the masters...

I'm about to release masters to three different artists/labels. I was asked by one of them if I had a delivery checklist for them to sign. This is a good idea. I could probably draft up something weird, or ask my lawyer, but I'd rather see what you guys do. Anyone got something that you have labels sign when you hand over the masters?

Most companies I have delt with issue a purchase order.
