Does anyone have a "contract" template for vocal/recording artists sessions?

I am basically looking for a contract/release form that states that the singer gives up all rights to the recorded material for X amount of money.

If anyone has a template to give out or know where I can find one - either PM me or post here.. Would save me LOTS of time to do this myself or get a lawyer to do it.

Doesnt have to be the most "PRO" written - aslong as it covers the basics in that she/he gives up all rights to the recorded material.
