Question LA4 wired to TRS "insert" cable

i need to wire some old LA4's to an unbalanced "insert" connector. this is what the LA4 back panel looks like. How would I wire this to a single 1/4" TRS insert?

i know "tip" is usually send from the board, and "ring" is return. just not sure how to connect to the LA4 itself.

sorry the pic quality is bad. it was taken with my phone. :(
