patchbay chart, form, spreadsheet???

Has anyone ever come across a good form (table, spreadsheet, ???) for use when laying out a patchbay? Every time I try to start one, I get caught up trying to decide between a simple list of points versus a graphical representation of how things flow. This isn't rocket science but I'm sure that somewhere along the line somebody has devised a really good chart that works better than most of the other lists out there. Anyone know of any good examples online? Thanks.

patchbay chart

Just to be clear, I'm looking for some help with laying out the patchbay overall orgainization. Printing labels are a long way off for me; and I've found that Excel works great for that task. I even use Excel to print console label strips, but you need a large format plotter to print out those 4 to 6 foot long strips.

My current problem is still the overall design of the patchbay and I was wondering if anyone has found a good format for the master chart or plan that lists all the connections, signal flow, etc.
