bad wires???

ok heres is my set up.....
akg c214 connected to the p-solo preamp(via xlr to xlr)
p-solo preamp then connects to my m-box mini (via 1/4 inch to 1/4 inch)

the other night while in a session we had alot of poping on some vocals we recorded with the preamp sounded like distrotion when the gain was turned up(at 12 oclock). i took the same 1/4 wire and used it for my monitor connection and my bx5a's had a horrible humming feedback sound when i turned them on....is this just a bad connection?....what can i do better?

PLEASE HELP

You might consider springing for one of these or these. Either one will make it much easier to troubleshoot problems of this kind.
