How to get rid of a ground loop ?

I experience a ground loop (low hum) in my B &W monitors. When connected just to the amp, they are quiet, the flowing hum starts when the amp is connected to Lavry DA. It is becoming a small phobia for me. Is there any way how to get rid of it in this very case ?
There is no hum as such coming from the Lavry - when I connect headphones there, it is perfectly silent.
An advice would be apprecciated

Are the amp and the DAC on the same AC circuit or separate ones?

You are using ballanced interconnects with the ground lifted on one side.... aren't you?

Originally it was not so (I put the amp in the other room and connected there - the hum was bigger), then I put both power cords (amp, DAC) to the same AC extension, the hum was less, but still there

I use a normal 4 m audio cable with balanced XLR Neutrik connections. Ground lifted on the side - does it mean any special adjustment ? (I have not made the cables and I am not expert in the subject ...)

Tiny is right, cut the ground (shield) on one side.
