Any insulation specialty shops will have it. Home Depot and Lowe's definitely don't carry it.

S.P.I.

I think it's "Specialty Products Incorporated". They are a commercial insulation supply company that, I believe, has locations around the country.

Home Depot and Lowes don't carry 3 lb or 6 lb density rigid fiberglass, at least in my area.

Insulation World - Thermal Insulation Products for Pipes, Equipment, Ducts - Asbestos and Lead Abatement Products for Industrial, Commercial and Residential Applications - Hopewell, Virginia

However, most any industrial insulation dealer will be able to help you. And yes, Lowes and Home Depot are worthless.

SPI $10 for 2x4' 4" thick sheets. Love what rock wool has done for both the control room and cutting room. Replaced all the Aurelex Lenrds in the corners and the front and back walls in the cntrl room. Can't believe how much the rooms have tightened up.

Do you just cover this with fabric and mount it to the wall?

Yup, the greater the surface area of the treatment is exposed, the greater it's effectiveness, as I understand it...but I would suggest checking in with the pros at John Sayers' or Ethan Winer's webboards.

The commercial insulators that cater to office - high rise construction use it all the time. Rock Wool - Curtainwall insulation - Fire proofing insulation are all interchangable names for the stuff on a construction site. I used to buy a truckload at a time for a high rise and it's super cheap, if you can make a deal with the commercial guys for his extras coming off a job it won't cost you much.

I got mine from Home Depot; they can special order it and it was cheap.

At least, the one near my place could!

I also got my Armstrong "painted nubby" acoustic ceiling tiles from them.
