Late night session listening on Headphones while still running the monitors thinking "why do my Headphones sound so strange?". Happens at least once a week :P
Was trying out a broadcasting amp someone gave to me in exchange for 705.
Unplugged a passive speaker I already had set up and plugged it into the new amp. Got mad that the amp was broken. Realized that I had plugged the new amp into the old amp.
- Accidently swapped AC adapters from a couple of MIDI sound modules. Both were 12 Volts == good. One was tip positive, the other tip negative == bad. Discovered the "Magic Smoke" preset soon afterward.
play back a sequence, close my eyes (to listen real close), and after 2-3 minutes saying to myself "this should change up any minute now"... only to find out that the friggin loop button is active.
I've also fallen asleep while working on crazy long ambient synths/grooves.
I have the dumb habit of hearing a mixdown/song im working on with a vinyl record playing at the same time.
Its to SEE if the music looks nice on the record lolol

That's a great idea, and I think not dumb at all. I can imagine it helps you listen to your music like it was from someone else, much more objective
I backed my chair up and smashed my Kawai DC-8 ram card for my K1r.
Luckily I didn't pay the insane ebay price for it. (I got it for around $15) Then I found a sysex bank dump program so I can back up patches if my K1r ever becomes full. At the rate I program the thing that would take years.
Used to keep my Cubase dongle on a usb port on the FRONT of my PC, right by my legs. Gave it a good kick by mistake one time and, though it still works somehow, its now kinked at a 40* angle lol
Man, I lost my whole studio by doing this. Beware. Credit will bite you in he ass, and interest is the devil. It eats up money that could be used to buy even more gear. Now I pay cash for everything.

The worst thing i do on an all too regular basis is knocking my guitar into another one of my guitars... Double fail!
I have just started learning the drums so I force every guitarist who comes into the studio to play Seven Nation Army for me to practice along with at the end of a session.
Most of them think it's pretty funny.
I also sometimes accidentally leave the fan heater on when recording.
Save a new project that has the potential to become an awesome song all in one file, so my computer can crash in the middle of saving and corrupt the file, causing me to contemplate suicide.

Oh I wouldn't worry about that!! There is a certain film soundtrack with a load of talking left in under a string cue that I only noticed AFTER release!! Fortunately no-one else did
