I tend to start a new thing from scratch, record a bunch of tracks in the heat of the moment, come up with some fancy instrument parts, and forget to take any notes whatsoever on what patches or effects settings I used, and also forget to write down the fancy parts. Then when I continue I have to re-record parts because I no longer remember the sound parameters, and possibly also transcribe them first because I no longer remember how to play them.
This was years ago in the Mac Plus era of "Desk Accessories" and "Font/DA Mover" (1980's), but....

Complain about the mess I have in the back corner of my studio but never put those damn carton boxes and bags full of cables in storage.
Realize I the synth part needs reverb, but then leave the loop running half hour while I contemplate the sunset I see from my studio window (happens when I’m stoned). The transition from blue to purple of the sky during sunset is very special to me, so I easily get distracted by it.
After finishing contemplating the sunset (in the meantime it’s almost dark outside) I’ll record a conga loop to spice up the groove, or add some other synth parts, record the song and call it a day.
Listen to it at home and realize the synth part is actually dry and need some reverb.
Forget to disengage the MONO button on my desk at the end of the session, then come back one day later and wonder what’s wrong with the pan pots (mixer sold, this shouldn’t happen anymore)
The dumbest thing i do is record at 16-bit 48000hz on a Soundblaster Audigy soundcard like it's still 1999. Maybe i'll buy something decent one of these days! To be fair, the drums are tracked on a Korg D888 thing which may have better quality inputs (iunno?!), and most of the synth and guitar parts are loaded with effects which might cover up the inherent ass-ness of my recordings.
Those Akai EIE I/O boxes look kinda nice, I like a VU meter.

..not finish a track and save it to the folder with hundreds of other patterns that aren't complete. Start a new pattern and do the same thing over again.
Lately I've been suffering from some extreme writer's block - for about a year. I wrote and recorded 6 albums in two years, and now...I've got nothin'. However, I'm still playing music almost every day.
But I'm always starting albums that go no where, so in my main directory I have like 15 files called some variation of NEW ALBUM! and all of them are comprised of one lame track of some stupid MIDI sequence. And I keep doing it, over and over. Shit, I did it last night, and I'll probably go home tonight and hate what I did.
This weekend I recorded a vocal on a new condensor microphone.
Upon playback it sounded just awful. A waste of money.
Then I worked out I had the mic facing the wrong way, so I was singing into the back of it.
So if you want to record a lo-fi vocal, this is a good way to do it.

To me this happens when playing MIDI like hell on a buss

Yeah, it only really happens to me when I cover the big LCD of the keyboard with something as that is the only indicator that power is on and I am so used to never to turning it off.
-Cool, sampler is on, but no MIDI light flashes, hmmm...
-Press more keys, scratch head.
-Make sure DAW routing is correct. It is, still no signal.
After I've followed every cable, I pick up my phone to check some email and there it is.. a big glaring LCD that is OFF.... EL FFFUUUUUUUUUUAAAAAA
