I used to work for Molinare and we had the exact same issue all the time. All dubbing theatres were Audiofile-based. That was in 2006 (not 1996!) After quite a bit of research I'd had success going PT->Audiofile with Pro-Convert (it was called Cui Bono Soft EDL Convert back then). We've trialled all sorts of EDL converters in the past and had no luck, but this was the only one that transfered the fades correctly. Haven't used Audiofile since 2007, so things might have changed. But as far as I remember, I used to export a Pro Tools 5 session, bring it into Pro-Convert and export to OMF 1.0. There might have also been an option to make it AMS Audiofile compatible or something similar. Only linear fades, but it was fairly straightforward and worked fine.
I've heard Molinare have now upgraded all their suites to Pro Tools, but it's amazing how some of the big post houses in London still save project files to floppy disks in 2010...
