When it comes to attaching lav´s, I´ve had some good experience with these: DPA DMM0509 Miniature Concealer | DV247
Attached on the skin of the actor, or on the t-shirt/shirt/dress or whatever the actors are wearing. Use medical tape to make it stick and not move. Lav´s needs constant attention, I usually check up on them between every shot. It´s usually trial and error to find the right place for the lav´s.
