You have another problem: you can't know, with standard tone test, if that deep is real. When we have an earing loss, it's not only one frequency but a group of frequencies around the deepest. So the 4 kHz could be the central frequency of the one of the group, but you can't tell which is without a better audometer to check the frequencies around 4 kHz. Think to a bell filter with high Q with attenuation,
