A friend just told me that Greg Fidlman (who mixed Death Magnetic) routinely has a limiter on the master bus. And on a recent project, that friend had asked Greg to reprint mixes again without the limiter because the first batch was incredibly brickwalled. So I guess the stage where it all went wrong was indeed the mixing.
