Serious limiting and maximising plugs like the Sonnox or our DSM have dithering included - usefully selectable for 16bit CD mastering as well.

There is no actual noise increase from dither in practical terms - all it does is turn harmonically related errors (distortions and error stuff) into noise. In other words in the presence of any signal, the harmonically related errors would be just as large as the dither noise anyway.
