Well, I can only speak for myself, but AFAIK, any pronounciation sounds about right to my ears. I'd say MeeDee (as in CD) if it was a separate word, but maybe not if it was in conjunction with something else; say "Middee Keyboard" for instance (Middee as in "Kitty"). But then I'm not British.
Anyway, in Swedish MIDI is pronounced as CD.
